ACPM Health Systems Transformation     Learning Institute



The Health Systems Transformation (HST) Learning Institute, held in conjunction with ACPM’s Preventive Medicine 2016 annual conference, was a huge success with participants including preventive medicine physicians and other healthcare and public health professionals. The institute covered timely topics including:

  • An overview of HST efforts at Centers for Disease Control and Prevention (CDC),
  • Role of preventive medicine in HST,
  • An overview of Alternative Payment Models (APM),
  • Principles of team-based care and the role of Community Health Workers, and
  • Health information technology and its role in preventive medicine and population health.

ACPM recruited an exciting roster of speakers experienced in integrating primary care and public health. The speakers provided a strong foundation in HST, through case studies, and helped the participants to understand the challenges and opportunities for preventive medicine in transforming the future of healthcare and health systems towards population health and disease prevention. The participants took part in two skill-building exercises to improve their understanding of the complex nature of our health systems: they learned how to build an Accountable Care Organization (ACO) and how to develop a team-based care delivery model to address the prevalence of asthma in Detroit.

  • Obtain competency-based knowledge and describe the goals in several aspects of Health Systems Transformation including Alternative Payment Models, structure design and function of an accountable care organization, team-based care and Health Information Technology.
  • Identify the unique role that preventive medicine physicians can play to participate and advance their role in health systems transformation in various settings.
  • Understand the opportunities and barriers in health systems transformation through real-life case studies.
  • Describe characteristics of a transformed health care system (community integrated health care)



Below are a list of reading materials that we have compiled. Participants are strongly encouraged to read these materials; to familiarize themselves with various concepts within health systems transformation



The participants were divided into groups to participate in two skill building exercises: 

1. Accountable Care Organization (ACO)

Participants were provided an opportunity to understand the various components that go into building an Accountable Care Organization. Here is the FINAL ACO SKILL BUILDING EXERCISE

2. Team-Based Care Delivery Model
