ACPM is partnering with the American Medical Association and Black Women’s Health Imperative to support the development of new clinical and community-based approaches in addressing prediabetes in high-risk populations, with a priority on African-American and Hispanic women. We’re excited to be hosting an educational institute on diabetes prevention at our annual meeting, Prevention 2019, as part of this initiative.

Diabetes Prevention Institute attendees will have the opportunity to learn about the three health care organizations we have selected to receive grants to develop and test new models to increase prediabetes awareness, screening and referral of patients to diabetes prevention programs recognized by the CDC. These organizations will be using innovative practice protocols to tear down barriers to health and address prediabetes in high-risk populations.

Additionally, Institute attendees will learn about the National Diabetes Prevention Program from experts at the ACPM, AMA and CDC, with CME/MOC Credits available for institute attendees and have a first look at brand new online resources being released for providers working in this space. ACPM hosted three webinars on the Diabetes Prevention Program Demonstration Projects. The first webinar featured grantees from Independent Practice Associations. The second webinar featured grantees from Integrated Delivery Systems. The third and final webinar featured grantees from Federally Qualified Heath Centers.The webinars recordings below are eligible for 1.5 CME/MOC Credit.
