About the Program

The online course will provide a foundation in key concepts that are critical in moving health systems away from episodic, hospital and treatment-based care to a population health-based approach that incorporates resources from the community and which emphasizes health promotion, disease prevention, and health outcomes. The course will describe ways population health can be measured and explain the role of different entities in influencing population health. Through case studies and knowledge checks, physicians and health care leaders will be able to describe evidence based strategies that can be implemented to improve population health.
