Health care systems are continuing to transform from treatment and management of care to include prevention of disease and promotion of health at the population level. Additionally, there is a concerted effort from the public and the private sectors to reward physicians and allied health professionals based on value and quality of care rather than volume of care provided. Access the recorded webinar to learn about the significant changes that are being led by the Centers for Disease Control and Prevention (CDC), Center for Medicare and Medicaid Innovation (CMMI) and private health systems and how these changes can impact your practice of medicine at the individual and community levels.


At the end of the session, the participant will be able to:

  • Define health systems transformation (HST) as developed by ACPM, recognizeACPM’s role in advancing HST and additional opportunities for professional development.
  • Articulate the emerging changes in payment and delivery of care and the latest developments in the implementation of Medicare Access and CHIP Reauthorization Act of 2015 (MACRA) including Advanced Alternative Payment Models like the Comprehensive Primary Care Initiative Plus model and the Next Generation ACO model.
  • Discuss federal initiatives to address common health conditions through the integration of primary care and public health and evaluate the impact of these initiatives on the practice of preventive medicine
  • Identify the unique role that preventive medicine physicians can play to participate and advance their role in health systems transformation in various settings.
