STEP 2: CME surveys/MOC quizzes

  • To obtain CME credits, please log-in/register to complete a short evaluation survey for each session here.
  • To obtain MOC credit, please log-in/register to complete a MOC quiz for each session here.

All CME Evaluations and MOC questions require a log-in.  If you are not an ACPM member, please register to create a free non-member (guest) log-in account.

Once you have completed ALL required CME surveys and MOC quizzes, please submit a CME/MOC Request Form.

You may only complete this form if you already completed Steps 1 and 2.  F
orms will not be processed unless all surveys and/or quizzes are submitted.

STEP 4:  Payment

Below are the payment options you will see upon submitting the CME/MOC Request Form:

  • ACPM Member attendee: $0 per credit
  • ACPM Member non-attendee: $5 per credit
  • Non-ACPM Member attendee: $10 per credit
  • Non-ACPM Member non-attendee: $15 per credit
Once ACPM recieves payment, please allow up to 30 days to receive your CME Certificate via email. MOC credits are reported directly to the American Board of Preventive Medicine (ABPM). Please allow up to 4 weeks for these to show on the ABPM website.

CME/MOC Information



American Medical Association (AMA) Category 1

ACPM is accredited by the Accreditation Council for Continuing Medical Education (ACCME) to provide continuing medical education for physicians.  ACPM designates Preventive Medicine 2015 for a maximum of 29.75 Category 1 credits toward the American Medical Association (AMA) Physician's Recognition Award (PRA)™ Physicians should claim only the credit commensurate with the extent of their participation in the activity.

This Live activity, Preventive Medicine 2015, with a beginning date of 02/25/2015, has been reviewed and is acceptable for up to  29.75 Prescribed credit(s) by the American Academy of Family Physicians. Physicians should claim only the credit commensurate with the extent of their participation in the activity.

February 2015 through March 1, 2016. Original release date:  February 2015


Members please note, in order to receive complimentary CME/MOC credit you will need to log-in to the ACPM membership database prior to making payment. If you can’t remember your log-in information, please click here.

Once you have completed all of the steps, your CME certificate will be e-mailed to you. Please allow one to two weeks for processing. Your MOC credits will be reported directly to the American Board of Preventive Medicine. Please allow 4-6 weeks for these to appear on the ABPM website.
