Awards

Annually, ACPM recognizes individuals for significant contributions, dedication, and service to the College and preventive medicine. The Awards Committee accepts nominations in the Autumn and all award recipients will be recognized during the Preventive Medicine meeting in the Spring.

Award Nomination Process

  1. All candidates must be active ACPM Fellows.
  2. Check with the member to ensure they are an active ACPM Fellow and would be willing to accept the nomination.
  3. Submit a nomination letter, along with the candidate's curriculum vitae below. Both the nomination letter and CV are required for consideration. Website submissions are strongly encouraged.
  4. Nominations must be received by the deadline.

To submit a nomination, or if you would like to be considered for any of the vacancies listed below, please submit a nomination letter, with the proposed candidate’s curriculum vitae (CV) below. Both a nomination letter and CV are required for consideration. Please save the documents as a PDF or MS Word file, and upload them here.
