Annually, ACPM recognizes individuals for significant contributions, dedication, and service to the College and preventive medicine. The Awards Committee accepts nominations in the Autumn and all award recipients will be recognized during the Preventive Medicine meeting in the Spring.
Award Nomination Process
Nomination Process: The nominator is responsible for submitting a complete award nomination packet. Because nomination materials are to be uploaded below, you will need to have access to electronic copies of the nominee's CV/resume and letters of support.
A complete nomination packet must include the following items:
All required fields typed on the official nomination form below
A narrative letter of justification (no more than two pages) from the nominator describing the most significant contributions of the nominee and an explanation of how the nominee meets the award criteria
At least one additional supporting letter (no more than two) (Only for Federal Award)
Current complete curriculum vitae or a detailed resume of the nominee that includes professional activities, publications, and awards/honors
Nominations must be received by the deadline of December 2, 2016.
