2015 ACPM HEALTH SYSTEMS TRANSFORMATION REGIONAL MEETINGS

 

Regional Meetings Overview: 

ACPM hosted two regional summits to highlight health systems transformation activities and the role of preventive medicine in this work. Presenters included state officials and stakeholders, state Departments of Health, ACPM members involved in health systems transformation work, U.S. Centers for Disease Control and Prevention (CDC) Office of State, Tribal, Local, and Territorial Support staff, U.S. Centers for Medicare and Medicaid Services (CMS) Center for Medicare and Medicaid Innovation staff, and health insurance partners among other experts. 

ACPM defines health systems transformation as systems-based approaches to improving population, community, and individual health by incorporating and addressing the determinants of health and increasing the efficiency and effectiveness of health care. 

Summit locations were chosen based on their involvement with health systems transformation projects, proximity to a Preventive Medicine residency program, and their focus on population health. A minimum of one hour of the summit was set aside as a Preventive Medicine Grand Rounds, targeting preventive medicine and other physician residents, medical, and public health students. 

Learning Objectives: 

  • Identify innovative health systems transformation activities occurring in the surrounding region
  • Discuss the role of population health in health systems transformation
  • Define the roles of public and private sector entities in health systems transformation work

 

Western Regional Health Systems Transformation Meeting:

The Western Regional Summit occurred on May 12, 2015 in Sacramento, CA and included representatives from CA, WA, OR, and NV.

Northeastern Regional Health Systems Transformation Meeting

ACPM hosted the last Health Systems Transformation Regional Meeting on June 18, 2015 in Albany, NY. ACPM convened leaders and members from MA, ME, NY, VT, and PA to share insights, exchange ideas, and learn from each other as we explored the role of preventive medicine in this new era of healthcare delivery. The successful summit combined highly interactive sessions from state public health officials from New York, Vermont, and Massachusetts who shared their State Innovation Model award experiences and the private and philanthropic sectors with presentations from the New York Academy of Medicine, the Commonwealth Fund, the Xerox Corporation, the Finger Lakes Healthy System, and the Maine Health Management Coalition.  Participants also heard from Ursula Bauer, PhD, MPH, Director of the National Center for Chronic Disease Prevention and Health Promotion, CDC on the how HST plays a role in the integration of primary care and public health. ACPM thanks our members and co-sponsors for their work in helping to organize, moderate, and speak at the Northeastern Regional Summit

ACPM Members: $5 per credit hour

Non-Members: $15 per credit hour

 

Once you have completed all of these steps, your CME certificate will be e-mailed to you. Please allow up to 5 business days for processing. Your MOC credits will be reported directly to the American Board of Preventive Medicine. Please allow 4-6 weeks for these to appear on the ABPM website
