|Addressing CDC's 6-18 Initiative - A Health Systems Transformation Project|
ACPM has recently received a grant from Centers for Disease Control and Prevention’s (CDC) Office of State, Tribal, Local and Territorial Support (OSTLTS) to address HST through CDC 6|18 initiative.
Through 6|18, CDC is targeting six common and costly health conditions – tobacco use, high blood pressure, healthcare-associated infections, asthma, unintended pregnancies, and diabetes – and 18 proven specific interventions that formed the starting point of discussions with purchasers, payers, and providers.
ACPM is currently developing year-long activities to promote this evidence-based initiative to preventive medicine physicians and understand the challenges,opportunities and the unique role that preventive medicine physicians can play in implementing these initiatives to lower the burden of these health conditions.
Survey on CDC 6|18 Initiative
As physicians and health professionals working in prevention, many of you are engaged in addressing one or more of the six conditions on either an individual or population level. We wish to learn more about the evidence-based interventions that you use, promote or research to address any or all of these conditions. In October 2016, we developed a survey to learn more about preventive medicine physicians' challenges and experiences in implementing the 6|18 initiative. We received a robust response from our members. We will soon upload a summary of the survey results.
Listening Sessions on CDC 6|18 Initiative
