Distinguished Federal Preventive Medicine Medical Officer Award

Description: The American College of Preventive Medicine Distinguished Federal Preventive Medicine Medical Officer Award recognizes a physician who, while in federal service, has distinguished himself or herself and the profession through his or her contributions to one or more of the fields of preventive medicine (Aerospace Medicine, Occupational Medicine, Preventive Medicine), resulting in significant and enduring improvements in the health of the population he or she serves.

Criteria: The nominee for the Distinguished Federal Preventive Medicine Medical Officer Award must be a physician who has served in federal preventive medicine practice for the past 10 years, is in federal service at the time of his or her nomination, and who has distinguished himself or herself and one or more of the fields of preventive medicine (Aerospace Medicine, Occupational Medicine, Preventive Medicine) by sustained outstanding performance and achievement. Applications will be evaluated based on the following criteria (listed alphabetically--not in rank order):

  • Active participation in Inter-Agency Collaboration (cross service activities)
  • Community service
  • Demonstrated leadership and mentorship
  • Impact on one or more of the fields of preventive medicine (Aerospace Medicine, Occupational Medicine, Preventive Medicine)
  • Innovation in practice, research, and or teaching
  • Sustained impact and contributions to positive health outcomes

Nomination Process: The nominator is responsible for submitting a complete award nomination packet. Because nomination materials are to be uploaded below, you will need to have access to electronic copies of the nominee's CV/resume and letters of support.

A complete nomination packet must include the following items:

  • All required fields typed on the official nomination form below
  • A narrative letter of justification (no more than two pages) from the nominator describing the most significant contributions of the nominee and an explanation of how the nominee meets the award criteria
  • At least one additional supporting letter (no more than two)
  • Current complete curriculum vitae or a detailed resume of the nominee that includes professional activities, publications, and awards/honors

The deadline for receipt of nomination materials is December 31, 2015
