Significant interest in communities in bringing primary care and public health together to improve population health has grown over the past 50 years, since the National Commission on Community Health Services "The Folsom Report" was released, calling for "communities of solutions." The key task now is to focus on the challenge of sustainable implementation of community-based models of primary care and public health integration.  

This pre-conference will expose learners to existing, successful models of primary care/public health
collaboration.  These models will be drawn from local and regional entities that have partnered with
community public health resources at various levels. Learners will be able to identify key concepts from the
programs highlighted and reflect lessons learned back to their own institutions and communities.
