Diabetes mellitus (diabetes) prevalence is high, affecting an estimated 15.7 million persons in the United States and climbing. Its cost impact was estimated in 1997 at $98 billion

Recommendations
The Task Force on Community Preventive Services makes the following recommendations based on the strength of the evidence of effectiveness found through the systematic literature review process

Health Care System Interventions

Disease management

Strongly Recommended

  • Disease management is an organized, proactive, multi-component approach to health care delivery involving all members of a population with a specific disease
  • Care is focused on and integrated across the entire disease and its complications.
  • The goal is to improve short-term and long-term health or economic outcomes for the entire population.

 

Case management

Strongly Recommended

Case management is a set of activities where the needs of a population with diabetes are addressed through improved planning, coordination, and provision of care

This usually involves assigning authority to one professional (the case manager) who oversees and is responsible for coordinating and implementing care

Self-Management Education

Community gathering places

Recommended

The Task Force recommends diabetes self-management education in community gathering places for adults with type 2 diabetes, and in the home for children and adolescents with type 1 diabetes based on the outcomes of glycemic control
