Enrolling in MOC Part IV

Step 1: Complete Form: Once you have completed the form you will be able to download your selected tool

Step 2: Download Selected Tool: You will select from Clinical Pediatric, Clinical Adult, 360, Report Card or Portfolio. Remember if at any time you would like to select a different tool, contact hbarno@acpm.org

Step 3: Pay MOC Part IV Application Fee and Download DPAP: At the bottom of the Tool Selection page, click, Finalize Registration. This will lead you to pay the ACPM MOC Part IV fees and you will be able to download your DPAP.
