Course on the CDC's 6|18 Initiative

ACPM has received a grant from the Centers for Disease Control and Prevention (CDC) Office of Health Systems Collaboration to develop a 1-1.5 hour(s) CME/MOC course on the CDC's 6|18 Initiative. It will provide an overview of the initiative and, through case studies, address the role of providers in implementing the initiative. This course will be available as a standalone elective within ACPM's E-Learning Portal..

SELECTION CRITERIA:
  • Expertise in at least one of the following areas: asthma, blood pressure, diabetes, tobacco use, or healthcare associated infections;
  • Experience in any of the following cross-cutting areas: provider engagement, changing provider behavior, team-based care, cross-sector collaborations, incorporating measures in EHR, and patient engagement and shared decision making
  • Able to develop, finalize and deliver 1-1.5 hour(s) CME/MOC course on the 6|18 Initiative.

WHY GET INVOLVED?

  • Research, develop and disseminate a course that highlights providers' roles in implementing the CDC's 6|18 Initiative;
  • Professional development;
  • Networking with other experts in the field;
  • An opportunity to help providers address and prevent chronic diseases including asthma, high blood pressure, diabetes, healthcare-associated infections, and tobacco use; and
  • Faculty will be compensated for developing the course.

WHAT IS THE 6|18 INITIATIVE?
The CDC is partnering with health care purchasers, payers, and providers to improve health and control health care costs. The CDC provides these partners with rigorous evidence about high-burden health conditions and associated interventions to inform their decisions to have the greatest health and cost impact. This initiative offers proven interventions that prevent chronic and infectious diseases by increasing their coverage, access, utilization, and quality. Additionally, it aligns evidence-based preventive practices with emerging value-based payment and delivery models.

The CDC is targeting six common and costly health conditions – tobacco use, high blood pressure, healthcare-associated infections, asthma, unintended pregnancies, and diabetes – and, initially, 18 proven specific interventions that formed the starting point of discussions with purchasers, payers, and providers. The number of interventions may fluctuate over time.
